Scattered-site Apartment Programs
BCHS offers two Scattered-Site Apartment Programs for 135 adults with serious and persistent mental illnesses in private apartments throughout the Clinton Hill, Crown Heights, East Flatbush, Flatbush, Fort Greene, Fort Hamilton, Park Slope and Prospect Heights sections of Brooklyn. By offering a rich continuum of services, such as on-going case management, daily living skills training, substance abuse counseling, and mental health care, we provide clients the intensive support they need to live independently.
The Transitional
Apartment Program places 63 individuals for a period of two to
three years with the goal of moving into more independent living situations.
The Supported
Apartment Program provides permanent housing for 72
individuals.
In order to be eligible for
admission to BCHS’ Supported and Transitional Housing, applicants must:
Be 18 years of age or older;
Have a primary Axis I
diagnosis such as schizophrenia, bipolar disorder, or major depression, and
either receives SSI or SSD due to mental illness, or has an extended
impairment in functioning due to mental illness, or relies on psychiatric
treatment, rehabilitation, and supports;
Be a documented
homeless individual as defined by the New York City Department of Homeless
Services (DHS) under the terms of the NYNY agreement, including an approved
HRA 2005 form and approval letter officially registering the individual as
NYNY eligible;
Demonstrate the ability
to live on their own or with roommates by maintaining psychiatric stability, managing
medication independently, being financially responsible and having shown a
commitment to sobriety;
Have a current
psychosocial assessment completed (conducted within the last 90 days);
Have a mental status
evaluation and a physical examination completed (conducted within the last
30 days);
Have documentation
verifying current income, or eligibility for Medicaid, HR, SSI or SSD;
Have copies of
appropriate identification.
In addition, Transitional Apartment Program applicants must have a documented need for at least five (5) Rehabilitative Services, which indicate impairment in functioning during the last 12 months or reliance on psychiatric treatment, rehabilitation and supports.
If the applicant seems
appropriate based on this information, a screening interview will be conducted
by BCHS Intake staff, usually at the applicants’ current housing location.
No applicant will be rejected on the basis of categorical criteria, such as
substance abuse history or participation in methadone treatment for opiate
addiction.
